Does SSM use MyChart? ›

Use your SSM Health MyChart account to book appointments, message your care team, view lab results, and more. SSM Health MyChart gives patients personalized and secure access to their digital health records.

If you try to log in unsuccessfully too many times, you might be locked out of the account. In this case, you won't be able to reset your password yourself. For help resetting passwords and regaining access to your account, please reach out to your healthcare organization's MyChart support desk.

Most likely, you have tried to access your MyChart account 5 times in a row using the wrong password. If you do this, for security purposes, your account is disabled. If your account becomes disabled, you should call 740-454-5008.

To have your account re-activated, you may contact MyChart Patient Support through our HELP page, or you may call our support line at 1-888‑97CHART (1-888‑972‑4278).

Note: MyChart is an Epic branded name. Your organization may use a different name for their patient portal. Access the application on your personal device. Carefully review the app's terms and conditions.

On your mobile device, open the Apple App Store (if you have an iOS device) or the Google Play Store (if you have an Android device). Look for one of the following icons to find the app store on your device: 2. Search for MyChart.

If you have an email address on file then your MyChart Username will be sent to your email account. If you do not remember any of this information, or you do not have a valid email address on file, you must call MyChart Technical Support at 216.444.1740 or toll-free at 1.866.915.3383 for assistance.

Your activation code will be printed on your after-visit summary, can be printed for you or sent to the email address we have on file for you, upon your request. You will not need to use this code after you have completed the sign-up process.

If you decline consent to share your information with affiliated partners, you will have multiple MyChart accounts: one for each organization, each with its own username and password. You will need to sign in to the specific MyChart account in order to view your information for that organization.

What happens when you deactivate MyChart? ›

You will no longer receive communications directing you to log in to MyChart. You do not need to be logged in to your MyChart account to have it deactivated. If you do not remember your username or password and want to deactivate your MyChart account, call or email your healthcare organization's MyChart help desk.

Check your email. We'll send a verification code to the email address you have connected to your MyChart account. Copy and paste the verification code into your MyChart log in screen, then proceed into your MyChart account.
